This webinar for physicians, nurses, and social workers reviews clinical implications for advance care planning and practical considerations such as consults, billing and referrals for document completion. It includes the advance directive mandate/law that requires the hospital to promote advance directives.

Learning Objectives
Upon conclusion of this activity, participants should be able to:
- Explain the advantages of advance care planning to patients, their family members and care team members
- Distinguish between advance care planning and advance directives
- Submit an order for an advance care planning consult
- Identify situations wherein advanced care planning discussions or consults are eligible for billing
- Cite billing processes to bill for advanced care planning consults or discussions using appropriate billing codes
