DESCRIPTION
Academic Grand Rounds at the Mayo Clinic Health System in Northwest Wisconsin is a monthly educational series centered on improving care and innovating healthcare delivery in departmental practices. Each month speakers provide contemporary updates to healthcare providers on evidence-based topics in nearly every specialty area imaginable. Sessions are held the third Friday of each month from 07:00 to 08:00 AM in the Luther Auditorium in the Mayo Clinic Health System- Eau Claire Hospital.
TARGET AUDIENCE
This course is intended primarily for physicians, nurse practitioners, physician assistants, and administrators in primary care (Family Medicine, Internal Medicine, Pediatrics, Urgent Care) along with those working in subspecialties.
LEARNING OBJECTIVES
Upon conclusion of this activity, participants should be able to:
• Describe how to provide consistent patient-centered care that improves the patients’ experiences with their providers.
• Demonstrate their ability to coordinate team-based care and improve systems.
• Describe how to provide cost-conscious medical care by decreasing low acuity emergency department visits and hospital readmissions.
• Apply team-based care concepts for optimal care for patients with chronic conditions.
